SRU Nomination Packet

The Soldier Recovery Unit Nomination Packet provides the standard operating procedures for reassigning Soldiers who need complex case management into the SRU program Recovery Unit. Please email SPC Jeffrey, Simone at simone.n.jeffrey.mil@health.mil, Ms. CornierMorales, Yanitza at yanitza.corniermorales.civ@health.mil and Ms. Blaser, Evangelina at evangelina.p.blaser.civ@health.mil or usarmy.tripler.medcom-tamc.mbx.sru-entrypacket@health.mil to request a blank nomination packet and nomination procedures or contact the patient administration department (PAD) at 808-655-0105. Completed nomination packets can be sent to usarmy.tripler.medcom-tamc.mbx.sru-entrypacket@health.mil

The goals of sending a Soldier to the SRU are to optimize medical, surgical, and psychiatric outcomes; prepare a Soldier for transition; and to prevent delays in clinical and administrative evaluation.
