Birth Certificates
The State Department of Health requires that all birth certificates be filed within one week of the child’s birth. Birth certificates are legal documents and as such must be letter perfect. To correct errors they must go through the Lt. Governor’s Office and could cost a minimum of $70 to correct. Parents should complete the birth certificate worksheet prior to the child’s birth and the worksheet should be brought to the hospital on the day of the delivery. The BC clerk will interview patients on the ward 8 - 9 a.m., Monday-Friday (except holidays). BC worksheets are available in the maternity handbook, at Admissions, the BC Office is located on the 1st floor, A-Wing, Room 404. The BC Clerk is normally available from 9:30 - 11:30 a.m. and 1:30 - 2:30 p.m. Monday through Friday, except holidays.
The Birth Certificate Clerk will provide a certificate of birth in order to update your personnel records and enroll the newborn child into DEERS temporarily. The active duty member must take this certificate to his/her personnel office. Certified copies of the birth certificate are needed to keep the newborn on DEERS permanently. You must request certified copies of the birth certificate from the State Department of Health, Vital Records Section, P.O. Box 3378, Honolulu, Hi 96801 at $10 for the first copy of each certificate and $4.00 for each additional copy of the same certificate ordered at the same time. It usually takes two months before certified copies are available. Call the Birth Certificate Clerk at 808-433-2031 if you have questions (i.e., unwed, divorced, alien, etc.).
