Patient Information

This page is designed to assist you in providing you additional information and links to Tripler and the Internet that we feel are a good source of information.  We will be adding form packages (physical, obstetrics, procedures, etc.) that you can download and complete at home prior to your appointment and save time during your visit. You can make appointments with the Family Medicine Clinic in one of two ways:

Sick call is no longer available. For continuity of medical care this is now an acute appt status, Active Duty Soldiers should call Central Appointments (808) 433-2778, lines open at 6:30 a.m.

You can leave telephone messages for your PCM by calling 808-433-2778.  Here you can ask your PCM the status of a result, renew/refill prescriptions, medical concerns, etc.  Your PCM or nurse will respond to your telephone call within 72 hours.

Tripler's Pharmacy Web site provides you with the hours of new and refill prescriptions and instructions for refilling your medicine.  Telephoned (808) 433-6962 refill prescriptions are usually filled within 72 hours by the pharmacy, excluding holidays and weekends.

We accept customer satisfaction feedback through our FOCUS "For Our Customer's Ultimate Satisfaction" Cards (located throughout the hospital) and through our Interactive Customer Evaluation (ICE) site located on the Internet at http://ice.disa.mil. If you have any questions, you may contact our Customer Relations office.
