The Soldier Recovery Unit's nurse case managers conduct a thorough and systematic assessment for all Soldiers assigned to the SRU. They are instrumental in developing an individualized nursing care plan in conjunction with the service member’s medical and Comprehensive Treatment Plan (CTP) goals.
Additionally, they provide feedback to service members on a weekly basis, and updates every 90 days with the command and treatment team, to ensure goals are both realistic and achievable for each Soldier.
Nurse case managers regularly collaborate with primary care managers, Licensed Clinical Social Workers (LCSW), and the Soldier’s assigned squad leader, to further develop the service member’s CTP.
Finally, nurse case managers facilitate other resources that support the clinical and administrative healthcare needs of the service member and family upon discharge from the SRU. This coordination ensures a successful transition back to duty, or on to separation/retirement with the Veterans Administration
