Nurse Case Managers
The Soldier Recovery Unit's nurse case managers conduct a thorough and systematic assessment for all Soldiers assigned to the SRU at Schofield Barracks. They are instrumental in developing an individualized nursing care plan in conjunction with the service member’s medical and Comprehensive Treatment Plan (CTP) goals.
Additionally, they also provide regular feedback to service members on a weekly basis, and again at 90 day intervals with the command and treatment team throughout the CTP process, to ensure their goals are both realistic and achievable based on the extent of the injuries.
Nurse case managers regularly collaborate with a primary care managers, Licensed Clinical Social Workers (LCSW), and the Soldier’s assigned squad leader, to further develop the service member’s CTP.
Finally, nurse case managers also facilitate the identification of resources that meet with the clinical and administrative healthcare needs of the service member and family upon discharge from the SRU, thus ensuring a seamless transition back to duty, or on to separation/retirement with the Veterans Administration as needed.
