Other pertinent information you would like to include

Unless otherwise requested by your provider, please bring only one (1) parent/guardian and the child being seen to the appointment. Please arrive no more than five (5) minutes before your appointment. Please remember to bring any paperwork requested with you to the appointment (intake packet, completed measures, exception to policy memo- if more family members need to be present). 

How to make an appointment

No referral required to request services through our clinic for you child. Please call our front desk at (808) 433-6418 to self-refer your child, to request a medication refill or to make a follow-up appointment.

2B Oceanside (take elevators to the second floor, turn left, follow the hallway until it ends at 2 double wooden doors)

FAQ's:

Q: Do I need a referral from primary care for my child to be seen?
A: No, you may submit a self-referral for your child by calling (808) 433-6418.

We accept customer satisfaction feedback through our FOCUS "For Our Customer's Ultimate Satisfaction" Cards (located throughout the hospital) and through our Interactive Customer Evaluation (ICE) site located on the Internet at http://ice.disa.mil. If you have any questions, you may contact our Customer Relations office.
